Two decades after creating the No Half Smiles concept, our founder and clinical director, Dr. Miguel Stanley, makes a return to this notion with an in-depth and interesting article that highlights another important return \u2013 that of dentistry to healthcare.<\/strong><\/p>

It was about refusing to do dentistry that only looks good on the surface while ignoring the disease underneath. It was about rejecting the idea that dentists should simply do what patients ask, instead of what they clinically need. And it was about restoring dignity to a profession that, in many parts of the world, has been reduced to transactional tooth mechanics.<\/span><\/p><\/div>

If you\u2019re a dentist reading this, I ask you:<\/span><\/p><\/div>

When did we stop being doctors?<\/span><\/p><\/div>

Imagine This in Medicine<\/span><\/h3><\/div>

A patient walks into a cardiologist\u2019s office. He needs a triple bypass. Instead, he says:<\/span><\/p><\/div>

“Doc, I\u2019m on a budget \u2014 could you just do the one artery that\u2019s really blocked? I\u2019ll fix the others later.”<\/span><\/p><\/div>

The cardiologist would refuse. It would be unethical. Dangerous. Illogical.<\/span><\/p><\/div>

Or imagine a woman visiting an oncologist. She\u2019s been diagnosed with breast cancer in both lobes. She says:<\/span><\/p><\/div>

“I have a wedding next week. Can we just treat the left one? The right doesn\u2019t hurt yet.”<\/span><\/p><\/div>

No physician would agree. The cancer doesn\u2019t care about her schedule.<\/span><\/p><\/div>

But in dentistry? This happens every single day.<\/span><\/p><\/div>

Patients come in with abscesses, failing root canals, systemic infections, and a collapsing bite \u2014 and say, <\/span>\u201cCan you just fix the front tooth? I don\u2019t like the way it looks.\u201d<\/span><\/p><\/div>

And the dentist, afraid to lose the patient (and the income), says yes.<\/span><\/p><\/div>

That is <\/span>not<\/span> healthcare.<\/span><\/p><\/blockquote><\/div>

That is <\/span>not<\/span> ethical.<\/span><\/p><\/blockquote><\/div>

That is <\/span>not<\/span> dentistry.<\/span><\/p><\/blockquote><\/div>

The Dentist as Healer \u2014 Not Technician<\/span><\/h3><\/div>

A healthy mouth is the foundation of systemic health.<\/span><\/p><\/div>

And yet, in most hospitals today, a full-body checkup doesn\u2019t even include a dental screening.<\/span><\/p><\/div>

You\u2019ll get your heart scanned, your lungs checked, your blood analyzed, and your hormones tested. But no one checks your oral microbiome, your jawbone, your root canals, or your occlusion. And yet, research for over 30 years has shown clear links between periodontal disease and cardiovascular risk, neuroinflammation, autoimmune burden, even poor sleep and fertility issues.<\/span><\/p><\/div>

Dentistry has been exiled from mainstream healthcare \u2014 and dentists have let it happen.<\/span><\/p><\/blockquote><\/div>

No Half Smiles: A Treatment Philosophy<\/span><\/h3><\/div>

The <\/span>No Half Smiles<\/span> philosophy is built on a simple but powerful idea:<\/span><\/p><\/div>

Never do what the patient wants until you’ve fully explained what they need.<\/span><\/p><\/blockquote><\/div>

It\u2019s okay if the patient can\u2019t afford it all right away. But what\u2019s not okay is to ignore pathology just to make a tooth \u201clook good.\u201d If your treatment doesn\u2019t start with diagnostics \u2014 if it doesn\u2019t prioritize biology and long-term function \u2014 then you\u2019re not doing healthcare. You\u2019re decorating dysfunction.<\/span><\/p><\/div>

At my clinic, the White Clinic in Lisbon Portugal , we break every treatment plan into three domains:<\/span><\/p><\/div>

1. <\/span>Biological<\/span><\/a> \u2013 infections, cysts, gum disease, bone lesions, toxic materials<\/span><\/p><\/div>

2. <\/span>Mechanical\/Functional<\/span> \u2013 occlusion, missing teeth<\/a>, orthodontic issues<\/a><\/span><\/p><\/div>

3. <\/span>Aesthetic<\/span><\/a> \u2013 alignment, color, shape, smile design<\/span><\/p><\/div>

We always treat in that order. Because beauty without biology is a lie. It’s been this way for decades.<\/span><\/p><\/div>

I\u2019ve turned patients away who refused to treat infections but insisted on cosmetic veneers. And yes, some went elsewhere. But many came back \u2014 because deep down, they knew we weren\u2019t selling a service. We were offering healing.<\/span><\/p><\/div>

Dentistry Is Not a Marketplace<\/span><\/h3><\/div>

The problem? Many clinics now operate like marketplaces.<\/span><\/p><\/div>

Patients come in, point at what they want, and expect to walk out with it \u2014 like ordering from a menu.<\/span><\/p><\/div>

But you\u2019re not buying a handbag. You\u2019re entrusting your systemic health to someone trained to understand what lies beneath the surface \u2014 infections that don\u2019t hurt yet, bone lesions invisible to the eye, root canals that look fine on an x-ray but harbor pathogenic bacteria.<\/span><\/p><\/div>

Dentistry, it’s not a consumer product.<\/span><\/p><\/blockquote><\/div>

This is why at the White Clinic, every patient undergoes a full 3D CBCT scan, an a-MMP8 saliva test (an enzyme present in saliva when there is a lot of collagen degradation due to poor health) as well as a Vit D test<\/a>. I remind you that vitamin D is critical<\/a> for bone remodelling and that your teeth are embedded in bone and we want it to be as healthy as possible. in our first appointments we don\u2019t guess, we diagnose. We don’t sell, we educate.<\/span><\/p><\/div>

And we say <\/span>no<\/span> when needed.<\/span><\/p><\/blockquote><\/div>

We\u2019re not in the business of doing one tooth at a time. We\u2019re in the business of <\/span>full oral rehabilitation<\/span>.<\/span><\/p><\/div>

Because that\u2019s where health lives.<\/span><\/p><\/div>

The Cost of Saying Yes<\/span><\/h3><\/div>

Years ago, I treated a powerful public figure who came in for a small filling. I noticed a hidden cyst on the adjacent tooth and warned him. He shrugged it off \u2014 <\/span>\u201cIt doesn\u2019t hurt. Just fix what I asked.\u201d<\/span><\/p><\/div>

Months later, at a high-profile dinner, he bit down on something and yelped in pain. In front of 80 people, he jokingly said, <\/span>\u201cStanley, you\u2019re a terrible dentist!\u201d<\/span> \u2014 assuming it was the tooth I treated. It wasn\u2019t. It was the one I warned him about.<\/span><\/p><\/div>

I lost credibility in that room. I lost trust. I probably lost future patients.<\/span><\/p><\/div>

And I realized: I will never do just what is asked of me<\/span>, <\/span>or<\/span> “half dentistry” <\/span>again<\/span>.<\/span><\/p><\/div>

Dentistry Deserves More<\/span><\/h3><\/div>

We need to bring dentistry back into the healthcare conversation.<\/span><\/p><\/div>

That starts with us.<\/span><\/p><\/div>

It means:<\/span><\/p><\/div>
